The Case Management Services Program consists of nurses and social workers providing case management services to children and adults with special needs.
Case Management Services help to assure coordinated, comprehensive services for children and adults. Within the Case Management unit are the Special Child Health Services Program, Early Intervention, the AIDS Community Care Alternatives Program (ACCAP) and the HIV Home Care Program. Early Intervention Services
The first three years of life are important, formative years in maximizing a child’s future potential. If you suspect that an infant or toddler may be experiencing developmental delays or may have a disability, seek help early. A referral can be made by calling the Regional System Point of Entry toll free number at 1-888-653-4463.
A service coordinator will talk with you about your concerns. If a developmental evaluation is needed, the service coordinator will work with the family to schedule a multidisciplinary evaluation of the child’s developmental levels and needs. Evaluations are provided at public expense with no cost to the family. If the child meets eligible criteria for the early intervention program, we will work to assure access and availability of individualized early intervention services.
