The OCHD Team Diabetes has two programs, one for individuals who are pre-diabetic and one for individuals who are diabetic.
Team Diabetes has graduated 56 diabetics through the Diabetes Self-Management Education and Support System (DSMES) programs well as conducted its first all-Spanish DSMES class.
The National Diabetes Prevention Program was started in 2024 and is off to a fast start. One of the keys to the program is healthy eating and our team will help you make healthy choices to take and keep the pounds off and live a diabetes-free lifestyle.
