High blood pressure and stroke are serious health conditions that can be easily managed by making small lifestyle modifications. Live Healthy Ocean County! (LHOC!) is a community-based chronic disease screening and education program provided by the Ocean County Health Department. Live Healthy Ocean County! provides Ocean County residents an opportunity to receive screening services, vaccinations, and medication management and education in a “one-stop shop” program where participants can learn the risk factors and prevention techniques of these common, but easily managed health conditions.

Some of our services include:

  • Blood Pressure Screening
  • Glucose Screening
  • Cholesterol Screening
  • Pulse Oximetry
  • Stroke Risk Assessment
  • Health Education
  • Diabetes Education

These programs are held at public locations throughout the County, on-site at adult community centers, clubhouses, and long-term care facilities, and at the Ocean County Health Department itself to make access to these preventive health services convenient and easily accessible to all of Ocean County’s adult residents.

CHRONIC DISEASE SELF-MANAGEMENT PROGRAM

Chronic long-term illness, such as heart disease, arthritis, diabetes or other health conditions are something we wish we could live without. Unfortunately, a majority of us will experience these conditions in our lifetime. This program is created to help you live with your conditions. Chronic Disease Self-Management allows you to take control of your health!

 

Diabetes Self-Management Education & Support

Diabetes Self-Management Education and Support (DSMES) is an accredited program for individuals living with diabetes and provides education to help manage diabetes. The program aims to improve health behaviors and health outcomes of individuals with diabetes. DSMES is four 2-hour sessions, once per week that covers the ADCES 7 Self-Care Behaviors (healthy eating, being active, healthy coping, taking medication, monitoring, problem solving, and reducing risks).

National Diabetes Prevention Program

The National Diabetes Prevention Program (NDPP) is a proven program to help prevent or delay Type 2 diabetes. The NDPP is a research based program focusing or healthy eating and physical activity. People with pre-diabetes who take part in a structured lifestyle change program can reduce their risk of developing Type 2 diabetes by 58% (71% for people over 60). Our NDPP program is a one-year program. We meet once per week for the first 6 months for one hour and for the 2nd six months, we meet once per month for one hour.

CHRONIC DISEASE HEALTH EDUCATION PROGRAMS

The Health Education department has many different programs that can help you with any chronic disease you might have and also help you prevent chronic diseases from happening.

Chronic disease programs available:

  • Medication management – Explains how to talk to your doctor, provides information on prescription safety, and how to take your medication correctly.
  • Sun safety – Provides information on ways to protect your skin from getting skin cancer.
  • Tobacco and smoke free- Provides information on smoking cessation and promotes a smoke free life.
  • Live Healthy Ocean County!- Provides education and screenings in a “one-stop-shop” program that is completely free of charge for blood pressure, stroke risk, and osteoporosis screenings.
  • Healthy Choices- Provides information on nutrition, cardiac health, and obesity for both children and adults.
  • Work wellness programs- Provides information on fitness and helpful tips for staying healthy and active in the workplace.
  • Testicular/ Breast Cancer Health- Provides education on testicular and breast health to prevent cancer. **Please note that the health education department can also develop or modify any program to fit your needs.
