Monitor health status to identify community health problems. What does this mean?  It is the responsibility of the local health department to do an assessment on the health of the community.  The OCHD is responsible for the Community Health Improvement Plan (CHIP) which identifies the priority health indicators for Ocean County with other community partners.  Many times this plan includes behavioral health and substance use as a priority area.  The OCHD has taken on other initiatives to assess the community such as the Ocean County Overdose Fatality Review Pilot Program.
